This survey is voluntary and should take up to 10 minutes.

From the CAP Project Website: “The Canadian Autism Partnership survey is for parents/family and caregivers of individuals with an Autism Spectrum Disorder, adults on the Autism Spectrum, professionals, service providers and organizations working with an individual or multiple individuals with an ASD, as well as, interested members of the general public.  ASD includes Autism, Asperger Syndrome, PDD-NOS, Pervasive Developmental Disorder, and Autism Spectrum Disorder.

The Canadian Autism Spectrum Disorders Alliance (CASDA) and the National ASD Working Group would like to better understand areas of innovation and system barriers related to ASD in Canada, raise awareness and provide an opportunity for input into the proposed national model, known as, the Canadian Autism Partnership.

This survey is important because CASDA and the National ASD Working Group will use the information to determine the direction of the Canadian Autism Partnership Model and to inform the business plan that will be submitted to the federal Minister of Health in November 2016.”
